If you are leaving Cleveland Clinic and have no intentions to be further associated, your CCLCM faculty appointment will terminate simultaneously with the last date of employment. Please email facultyaffairsCCLCM@ccf.org with the details.
Leaving Cleveland Clinic for a CWRU-Affiliated Hospital
Contact the faculty affairs office at the CWRU-Affiliated Hospital to have the CCLCM primary appointment transferred. CCLCM Faculty Affairs cannot facilitate the transfer.
Leaving Cleveland Clinic, but Continuing Academic Activities
For those leaving Cleveland Clinic but who need to continue mentorship or research, please immediately send an email to facultyaffairsCCLCM@ccf.org providing: (1) information on termination from Cleveland Clinic, (2) planned status change date, (3) anticipated continued academic involvement, and (4) a personal non-ccf email address.
Retiring from Cleveland Clinic
Retiring faculty with certain qualifications and over 10 years of service with CCLCM and/or CWRU can apply for a CCLCM Emeritus appointment. Please review the CCLCM Emeritus Application Instructions to see if you qualify. It is important to apply quickly before the faculty appointment is terminated.
Those that are retiring but plan to return as a retired contract or consultant staff; please send an email to facultyaffairsCCLCM@ccf.org stating (1) planned retirement date, (2) staff title for continued employment, (3) planned rehire date, and (4) a personal non-ccf email address.
